Homebase HRA
Homebase provides New Yorkers experiencing housing instability with various homeless prevention services and aftercare services to families and individuals exiting NYC DHS shelter to permanent housing. Homebase locations are servicing clients either via phone or for in-person services. Anyone seeking Homebase services should call the office in advance to schedule an appointment.

Homeless Prevention Fund-DHS
The Homeless Prevention Fund provides emergency financial assistance to households
citywide who are unable to secure sufficient assistance through available programs and are at
imminent risk of homelessness due to rent amounts. The Fund offers emergency financial
assistance to low-income households who meet all of the following criteria:
1. Household income range between $15,000 and $30,000 annually
2. An eviction petition has been filed
3. The household has the ability to pay rent in the future

CityFHEPS
CityFHEPS is a rental assistance supplement to help individuals and families find and keep
housing. It is administered by the Department of Social Services (DSS), which includes both the Department of Homeless Services (DHS) and the Human Resources Administration (HRA)
CityFHEPS – HRA.

Can I get CityFHEPS?
To be eligible for CityFHEPS, households must have a gross income at or below 200% of the
federal poverty level and meet one of the following four (4) criteria:
1. The household includes someone who served in the U.S. Armed Forces and is at risk of
homelessness; OR
2. The household gets Pathway Home benefits and would be eligible for CityFHEPS if they
were in DHS or HRA shelter; OR
3. The household was referred by a CityFHEPS qualifying program, and DSS determined
that CityFHEPS was needed to avoid shelter entry; OR
4. The household is facing eviction in court (or was evicted in the past year)
AND:
– Includes someone who has previously lived in a DHS shelter; OR
– Includes someone who has an active Adult Protective Services (APS) case or is in
– A designated community guardianship program.

How to Get There: Subway: Take the 6 train to 28th Street. Walk east to 1st Avenue, turn left, and go north to 30th Street. Entrance is now at 30th Street and 1st Avenue

How to Get There: Take the 2 train to 149th Street, followed by the #55 bus to 166th Street and 3rd Avenue.

Prevention Assistance and Temporary Housing (PATH)

PATH is open 24 hours per day, including weekends and holidays.
PATH processes applications during business hours (9 a.m. to 5 p.m.)

The following forms of ID are very helpful during the intake process (but are not required):

  • Any form of ID with a picture and proof of age, such as a driver’s license, state-issued
    ID, passport or visa, welfare card or green card
  • Social Security card
  • Medicaid card, if available
  • If working, your most recent pay stub

Subway: Take the 2, 4, or 5 train to 149th Street/Grand Concourse Station. Head west on East
149th Street toward Grand Concourse. Walk north on Grand Concourse two blocks, to East
151st Street and turn left. Walk two blocks to Walton Avenue. The PATH office is located at the
corner on your right.
